The RN serves as the primary case manager, developing and implementing nursing plans of care for patients in their homes. Responsibilities include performing clinical assessments, carrying out physician orders, and maintaining accurate documentation.
Candidates must be a graduate of an accredited nursing school and hold a current Registered Nurse license in the State of Nebraska. BLS certification is required, while ACLS and previous home health experience are preferred.

Summary The Home Health & Hospice Registered Nurse makes home visits and develops a nursing plan of care and implements that nursing plan of care to patients at their place of residence in accordance with the philosophy, goals, and objectives of Cozad Community Health System.

Serves as the primary nurse/case manager for all assigned patients. Develops the plan of care for each patient, in coordination with the primary care provider, and oversees that plan of care for effectiveness.
Assesses patient’s condition at each home visit. Documents that assessment accurately and reports to the physician as needed. Provides direct patient care and demonstrates knowledge in home care/hospice as well as scientific and technical principles, drugs, and therapeutics. Carries out physician orders, dressing changes, venipuncture, home infusion, medication reconciliation, etc. as specified in the plan of care.
Performs and maintains documentation.
Follows all HIPAA guidelines and promotes and ensures patient safety.
Fulfill mission statement by performing job duties with a high level of customer service while contributing to a positive and professional environment.
Special projects and other duties as assigned.
Supervisory Responsibilities No supervisory responsibilities.
Education and/or Experience Graduate of an accredited school of nursing. Previous Home Health & Hospice experience preferred. Electronic Health Records experience and management experience preferred.
Licensure/Certification Current Registered Nurse licensure in the State of Nebraska. Basic Life Support (CPR) Advanced Cardiac Life Support (ACLS) preferred Valid Driver’s License
Work Environment While performing the duties of this job, the employee regularly works in a medical clinic setting and patient residences. The noise level in the work environment is usually moderate.
