The Licensed Practical Nurse will perform assigned duties during home visits, including medication administration, wound care, treatments, and monitoring client conditions, while reporting changes to management. Responsibilities also involve following up on doctors' orders, performing client assessments, case management, and accurately documenting all care provided using electronic medical records.
Candidates must possess a current Licensed Practical Nurse license in the state of Delaware and have a minimum of one year of recent, verifiable experience as an LPN, graduating from an accredited nursing program. The role requires the ability to work independently, strong interpersonal skills, solid computer skills, and the ability to travel to assigned cases.
BAYADA by Bayhealth Home Health Care is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in our office located in Dover, DE. This office services our adult clients on a per visit basis in homes throughout Georgetown, Millsboro, and Dagsboro. We have full-time or part-time positions available.
One year prior clinical experience as a licensed Practical Nurse is required.
As a home care nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.
