The role involves providing hands-on patient care within the scope of an RN license, including complex procedures like PICC lines and wound care, while implementing and documenting the plan of care directed by the Case Management RN. Responsibilities also include patient and family education, coordinating with other disciplines, and participating in on-call duties as required.
Candidates must be a graduate of an accredited school of nursing with a current California Registered Nurse License and a valid CA Driver's License. A minimum of one year of recent nursing experience is required, though this may be waived for internal candidates or new graduates, with home health experience being preferred.
