The role involves implementing daily telephonic case management interventions for high-risk members by assessing needs and coordinating discharge plans with clinical teams while following members in the acute inpatient setting. Key duties include evaluating discharge needs, coordinating post-discharge care with providers, participating in rounds, and arranging necessary post-hospital services.
Candidates must possess a Bachelor of Science in Nursing and hold a current, unrestricted Compact RN license in Tennessee, along with a minimum of five years of recent acute inpatient hospital experience. Required experience also includes at least two years in discharge planning or case management, and proficiency working with various health insurance products like Medicaid, Medicare, and Commercial plans.
Bachelor of Science in Nursing Current, unrestricted Compact RN license in the State of Tennessee 5+ years of recent experience in the acute inpatient hospital setting 2+ years of experience of discharge planning and/or case management Experience working with multiple health insurance products (Medicaid, Medicare, Commercial) within the insurance industry, including regulatory and compliance requirements Proficient in typing skills and software applications that includes, but is not limited to, Microsoft Word, Microsoft Excel, Microsoft PowerPoint and Microsoft Outlook Designated work space and access to install secure high speed internet via cable/DSL in your home Permanent residence in the State of Tennessee
