The Nurse Practitioner will provide comprehensive assessment, intervention, education, preventative, and urgent care services for high-risk patients in their homes, acting as an extension of the Primary Care Provider and Clinic Care coordination team. Responsibilities include managing complex, acute on chronic, and serious illnesses by developing and overseeing home-based care plans, and treating non-life-threatening medical problems to avert ED or inpatient visits.
Candidates must possess experience in Home Care, Hospice, or Acute Care, along with a current and unrestricted Nurse Practitioner license in Iowa, including state and federal authority to prescribe medications. Excellent communication skills are necessary for effective interaction with patients, physicians, and staff.
Provide Comprehensive assessment, intervention, education, preventative, and Urgent Care services for high-risk patients their home environment. Provide alternate ambulatory-based healthcare to avert patients need to be seen in the ED, IP, or SNF Work collaboratively as an extension of Primary care provider and Clinic Care coordination team Assist patients with complex, acute on chronic, and serious illness with developing, implementing, communicating, and overseeing the patients’ home-based care plan Collaborates and consults with the patient/family and health care team members Treat non-life-threatening medical problems or conditions that could get worse if a patient were to wait to be seen at a clinic Medical assessments and treatment planning Home medical safety assessments Medication review and supportive management Lab draws associated with medical assessment and monitoring Xray or ultrasounds associated with medical assessment and monitoring Provide the best outcome for every patient every time
Home Care, Hospice, or Acute Care experience required Hold a current and unrestricted license in Iowa Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary Employee maintains current licenses and/or certifications required for the position.
