The role involves conducting psychosocial assessments of patients and caregivers to identify emotional, social, and environmental needs related to their diagnosis and developing, implementing, and evaluating care plans. This includes providing counseling, education, resource referrals, and collaborating with interdisciplinary teams while maintaining patient dignity.
Candidates must possess a Master's degree in social work from a CSWE-accredited institution, or a Bachelor's degree if working as a BSW, and must be licensed as an LCSWA or LCSW by the NC Board and potentially hold LISW-CP in SC. Medical social work or hospice experience is preferred, and computer proficiency is required.
