The Hospice Medical Social Worker is responsible for providing consultation and services to homebound individuals facing complex social and emotional problems that interfere with managing their medical regimen. This role involves delivering one-on-one, relationship-driven care to make a real difference in the lives of patients and their families.
Candidates must possess a Master's Degree from a Council on Social Work Education accredited school and hold a current State of Michigan Social Work License. A minimum of two years of social work experience in a healthcare setting is required, along with the ability to serve acutely and chronically ill patients.
