The Behavioral Health Counselor provides professional behavioral counseling services to designated patients, including counseling and discharge planning within a hospital or outpatient setting. Responsibilities involve assisting patients/families with physical, emotional, and social concerns, completing required documentation like treatment plans and progress notes, and coordinating care with community providers.
Candidates must possess a Master’s degree from an accredited graduate social work or counseling program, coupled with a minimum of five years of experience in a behavioral health-related field. Licensure as an LCSW, LPC, or LMFT is mandatory for this advanced practice role.
Provides professional behavioral counseling services to designated patient population, and provides counseling and discharge planning as well as other behavioral counseling services to patients and families. The general practice of behavioral counseling in a hospital or outpatient setting includes interviewing, assessment, planning, intervention, evaluation, case management, mediation, supportive counseling, information and referral, problem solving, client education, and advocacy. This position has documented advance practice skills via certification and experience.
Assists patients/family/significant other with physical, emotional, and social concerns that arise during the episode of care. Provides treatment/discharge planning services to patients. Completes required documentation including, treatment plans, safety plans, and progress notes. Completes initial assessment and continued documentation to obtain pertinent background/social/ financial data. Documents social determinants of health as indicated in medical record. Connects patients/family/significant other with community resources to aid in their treatment and recovery. Coordinates care with community providers as needed. Acts as facility liaison with adult and children protective services as indicated. Cooperates with facility staff and physicians and keeps them informed of progress with patients via verbal communication and chart documentation in medical record. Exhibits the skills of empathy, communication, organization, critical thinking, active listening, and patience. Provides oversite in a situation where a licensed individual is required. Provides expert training and supervision in the field to others, as requested. Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested. Performs other duties as assigned.
Minimum Education: Master’s degree from accredited graduate social work or counseling program. Minimum Experience: Five (5) years of experience in a behavioral health related field. Licensure Requirement: Must be licensed as LCSW, LPC, or LMFT.
