The role involves providing ongoing support and expertise through comprehensive assessment, planning, implementation, and evaluation of individual patient needs. Core duties include coordinating discharge planning to ensure patients progress to the least restrictive environment and managing hospital activities related to case management.
A minimum of two years of utilization review or case management experience, or social work experience, is required. Candidates must maintain current professional licensure in nursing or their professional field of certification, with preferred qualifications including an Associate's or Bachelor's degree in a healthcare-related field.
Reports to and is under direct supervision of Case Management Department. Provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. Promotes continuity of care and cost effectiveness through the integrating and functions of case management.
• Minimum of two (2) years utilization review/case management experience or social work experience.
• Maintain current professional licensure in nursing or professional filed of certification.
• Appropriate certification in the case management preferred (e.g. Commission for Case Management Certification (CCMC) or Association of Rehabilitation Nurses).
