As a Licensed Practical Nurse in Long-Term Care, your key responsibilities will center on delivering person-centered nursing care, closely aligned with each resident’s care plan. You’ll administer medications and treatments according to physician orders, monitor vitals, observe changes in condition, and document findings clearly in electronic health records. You’ll perform routine assessments, assist with activities of daily living, support wound care and catheter care as needed, manage acute symptoms under direction, and collaborate with registered nurses, physicians, and therapists to update care plans. You’ll educate residents and their families about medications, safety, fall prevention, and healthy daily routines, empowering informed choices. Your role will also involve participating in rounds, contributing to discharge planning, and supporting transition care when residents move between levels of service.
