The Hospice Physical Therapist will plan, organize, and deliver therapy services focused on maintaining patient comfort, function, and quality of life, guiding patients and caregivers through supportive techniques and education. Essential functions include providing physical therapy services per the physician's plan of care, assessing functional status with a hospice focus, and instructing the team and family on therapy techniques and care goals.
Candidates must possess a Bachelor's or Master's degree in physical therapy from an accredited program and be licensed to practice as a physical therapist in the state of practice. Two years of relevant experience is preferred, and applicants must have a current CPR certification and a licensed, insured automobile.

Bachelor’s or master’s degree in physical therapy from an accredited program. Licensed to practice as a physical therapist in the state of practice. Two years of relevant experience preferred; home health or community-based experience helpful. Strong verbal, written, and organizational skills. Current CPR certification. Licensed driver with an insured automobile that meets organizational requirements.
