Professional who excels in this role: Effectively utilize strong clinical and assessment skills, cultural competence, and basic casework skills (referrals, advocacy). Provide therapy to assigned adult/geriatric patients per Behavioral Unit (BHU) policies, developing, and revising the treatment plan, utilizing an interdisciplinary approach, establishing specific short- and long-term goals. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of the patient and patient representative. Document the psychosocial evaluation, all one to one and group activities, therapies, and case management activities in the patient(s) chart. Documentation is accurate and meets all hospital, state and TJC requirements. Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health errors. Knows responsibility during a "Code Gray" and provides crisis intervention as necessary. Serve as a member of the interdisciplinary team participating in treatment team meetings evaluating and updating treatment goals and interventions. Provide/conduct group education and group programs per established schedule with emphasis on providing suicide prevention education to patients and/or patient representative. Serve as an advocate for the physical and emotional well-being of the patient protecting the patient's rights serving as a liaison between patients, referral sources, and community agencies. Maintain clinical and regulatory processes by demonstrating theoretical knowledge of psychopathology and a working knowledge of DSM-5. Perform utilization review process for inpatient admissions, participating in discharge planning including coordinating patient transfers to other facilities and coordinating community resources.
