The Physical Therapist will be responsible for evaluating patient mobility and function, developing and modifying treatment plans, and administering therapeutic exercises in a Long-Term Care setting. Key duties also include completing Medicare documentation and collaborating with care teams.
Candidates must possess a Bachelor's degree in Physical Therapy and hold a current state PT license, with strong knowledge of Medicare guidelines being essential. Prior experience in Long-Term Care is preferred, alongside effective communication and teamwork skills.
Physical Therapist (PT) - Long-Term Care LP Therapy LP Therapy is seeking a licensed Physical Therapist (PT) to provide skilled therapy services in a Long-Term Care setting focused on restoring mobility, strength, and function.
review physician referrals and medical records; evaluate strength, mobility, and functional ability; develop and modify treatment plans; administer therapeutic exercises and modalities; educate patients and families; complete timely Medicare documentation; collaborate with care teams; supervise PTAs and aides as required; support quality outcomes and productivity standards.
Bachelor's degree in Physical Therapy; current state PT license; LTC experience preferred; strong knowledge of Medicare guidelines; effective communication and teamwork skills. Work Environment: LTC facility; physical demands include lifting and mobility assistance.
