The Care Coordinator manages and coordinates care for clients in transitional and safe haven facilities by assessing needs, developing individualized care plans, and ensuring compliance with HUD grant requirements. This role involves extensive collaboration with providers and community resources to integrate all aspects of the client's care plan, including significant hands-on assistance with housing search and placement.
A Bachelor's degree in Social Work, Public Health, or a related field is preferred, alongside a minimum of two years of experience in housing services or case management. Candidates should possess strong organizational and communication skills, experience working with diverse populations, and knowledge of HUD regulations is preferred.
