The Inpatient Care Manager-MSW coordinates comprehensive care for a designated patient population by assessing needs, planning individualized care, and ensuring safe and timely discharge planning. This role involves acting as a clinician, care manager, and educator while collaborating with the team to achieve cost-effective outcomes and managing resources efficiently.
The minimum requirement is a Master’s Degree in Social Work along with less than one year of experience, potentially gained during an MSW internship program. Candidates must possess proven leadership skills and the ability to efficiently set priorities for care plan coordination.
Master’s Degree in Social Work. Less than 1 year experience In an MSW internship program. Ability to incorporate strategies for interacting with persons from diverse backgrounds. Ability to set priorities to coordinate care plans efficiently Knowledge of computer applications and analytical tools. Proven leadership skills.
Familiar with EHRs: EPIC. ACM - Accredited Case Manager - American Case Management Association within 3 Years or CCM - Certified Case Manager - Commission for Case Manager Certification within 3 Years Physical Demands Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
