As a Home Health Registered Nurse (RN), you will support patient recovery and empower patients and families to reach healthcare goals. You will also case manage and provide clinical oversight to an interdisciplinary team.
Requirements summary
Candidates must have an RN license in the state of Ohio and two years of experience as a registered nurse, preferably with home health experience. Current CPR and BCLS certifications are also required.

Responsibilities
Provide patient assessments and monitor health status. Use AI tools to streamline documentation for better focus on hands-on care. Ensure compliance with care plans and regulatory standards. Education and/or Experience Associate or bachelor’s degree in nursing required. Minimum of one (1) year nursing practice or equivalent experience required. Certifications, Licenses, and Registrations Active and unencumbered Registered Nurse license in state(s) of employment required.
