The Nurse Practitioner will conduct comprehensive assessments and manage members through acute medical situations or chronic condition exacerbations, including necessary testing and interventions. They will also ensure accurate documentation and collaborate with the interdisciplinary care team while providing counseling and education to patients and caregivers.
Candidates must graduate from an accredited Master of Science Nursing or Doctor of Nursing Practice program and hold an active, unrestricted Nurse Practitioner certification from a national board in a relevant specialty. Required qualifications also include active DEA licensure/prescriptive authority and reliable personal transportation.
