The PCP provides comprehensive, coordinated medical care for older adults, including evaluations, treatment, and care planning within an interdisciplinary team. They are responsible for executing Age-Friendly care and coordinating services across various healthcare settings and specialists.
Requires a valid Oregon license as a Nurse Practitioner or Physician Associate, a DEA certificate, and CPR certification. Candidates must have at least one year of experience with frail/elderly populations and five years of general healthcare experience.
