The Hospice RN Case Manager provides skilled nursing assessment, planning, and care to maximize patient comfort at the end of life. They coordinate services with an interdisciplinary team and supervise Home Health Aides and LPNs.
Requires a valid New York RN license, a valid driver's license with a reliable vehicle, and at least one year of acute care nursing experience. An associate degree is required, while a bachelor's degree or hospice certification is preferred.

Under the direction of the interdisciplinary team, the Hospice Nurse works under the general direction of the Hospice Medical Director and the attending Licensed Independent Provider (LIP) in the provision of skilled nursing assessment, planning and care to maximize the patient’s comfort at end of life and support the patient’s families/ primary caregivers. The Hospice RN is responsible for identifying patient/family needs and for providing supportive care in accordance with the attending LIP’s orders, the interdisciplinary plan of care and Guthrie Hospice policies and procedures. Responsible for providing direct supervision of Home Health Aides and or LPN’s providing care for Guthrie Hospice.

The pay range for this position is $39.00 - $54.12 per hour.
