The Social Worker will serve as the primary contact for an assigned panel of high-risk patients, conducting comprehensive psychosocial assessments and collaborating with interprofessional teams to deliver interventions and case management services. Key duties include providing compassionate end-of-life care support, managing longitudinal care, building patient trust, and identifying/addressing barriers to care through screenings and referrals.
Candidates must possess a Master’s degree in Social Work (MSW) or be a Licensed Clinical Social Worker (LCSW) in Florida, demonstrating strong clinical judgment and case management expertise. Requirements also include reliable transportation for travel to patient homes, self-motivation, experience working with diverse communities, and strong communication skills for both in-person and telephonic interactions.

The Social Worker role is ideal for a proactive, self-motivated, and compassionate professional who thrives in a collaborative care environment. This position works closely with an interprofessional care team to coordinate care and improve outcomes for a small panel of high-risk patients.
This is a hybrid role (50% field-based, 50% remote) focused on longitudinal care management. Standard working hours are Monday through Friday, 8:00 AM–5:00 PM, with flexibility as needed to meet patient care needs.
