The Care Coordinator will work directly with children, youth, and families in community settings, collaborating to connect them to necessary care and services. Key duties include coordinating care services like appointments and referrals, maintaining accurate client records, and communicating effectively with providers and partners.
Candidates must meet specific education and experience criteria, such as a High School Diploma with 3 years of experience, or a higher degree with less experience, preferably in children's behavioral health or related human services. A valid driver's license and reliable transportation are mandatory, and LSW/LPC licensure is preferred.
