The Mental Health Rehabilitation Specialist provides direct community support and case management services to assigned members using Psychiatric Rehabilitation principles and Recovery Philosophy. This role involves coordinating services, collaborating with various agencies, completing assessments, and providing crisis intervention and trauma-informed care.
Candidates must possess a Master's degree with two years of direct mental health service experience, or a Bachelor's degree with four years, or an Associate's degree with six years of relevant experience. A valid driver's license and willingness to use personal or company vehicles for client transport are mandatory requirements.

The Mental Health Rehabilitation Specialist provides direct members served community support and case management services to assigned members served using Psychiatric Rehabilitation principles, Recovery Philosophy, and Evidence-Based practices. This position provides education and support to members served and their families. S/he coordinates service needs and collaborates with other services and agencies such as the Conservator's Office, Public Defender's Office, Child and adult Protective Services, Probation Department, and Inpatient Services, Long Term Care Services, Residential Settings and County Mental Health Services. The Mental Health Rehabilitation Specialist completes assessments, provides mental health rehabilitation services, case management, crisis intervention, integrated whole person care, and trauma informed care.
Expected starting wage range is $26.46 - $32.69. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

Telecare’s community program in Central Orange County, Homeless General Population will focus on addressing the complex housing needs of up to 150 members, many of whom have long histories of homelessness and serious mental illness. The program will emphasize enhanced engagement and outreach efforts along with intensive case management, Vocational/Educational/Employment support and robust housing strategy services.
