The Medical and Behavioral Case Manager reviews cases involving members with acute or chronic conditions, collaborating with providers, patients, and internal resources to establish effective treatment plans within policy provisions. This role involves screening, assessing, developing case recommendations, educating patients and families, coordinating with providers, and continuously monitoring and documenting cases.
Candidates must possess a bachelor's degree in social work or a related field, along with a minimum of three years of experience in social work or a related area. Essential requirements include Social Work Licensure (LSW, LBSW) by the North Dakota Board of Social Work, advanced clinical expertise, and strong interpersonal and decision-making skills.

A day in the life
The Medical and Behavioral Case Manager is responsible for reviewing cases of members with acute or chronic conditions and collaborating with service providers, the patient and family, and internal resources to establish the most effective treatment plan within the provisions of the policy. This position screens, assesses and develops case recommendations, educates patients and family, coordinates with providers and monitors and documents case(s) on an ongoing basis.
Equivalent combination of education, experience or training determined to be acceptable by Human Resources may be substituted, unless regulated by contract or program standards
