The Care Coordinator acts as a central liaison coordinating care between residents, families, physicians, pharmacies, and home health/hospice providers to ensure continuity and quality of service. Responsibilities include communicating with physicians for medical orders, coordinating medications, following up on referrals, and maintaining accurate resident records in the EHR system.
A high school diploma or equivalent is required, though an Associate's or Bachelor's degree in a related health field is preferred. Candidates must possess strong organizational and documentation skills, knowledge of resident assessments and state regulations for assisted living, and proficiency with EHR systems.

As a Care Coordinator, you will serve as a central liaison between residents, families, physicians, pharmacies, home health agencies, and hospice providers to ensure continuity and quality of care. Responsibilities include communicating with residents’ physicians to obtain and clarify medical orders, accurately entering and revising physician orders, coordinating medication services with contracted pharmacies, and following up on home health and hospice referrals and documentation.
You will maintain accurate and timely resident records by scanning, uploading, and organizing documentation within the electronic health record system. Additionally, you will support the Director of Nursing with clerical, compliance, and regulatory documentation to ensure adherence to state and facility standards.

Compensation for roles varies depending on factors such as location, role, skill set, and level of experience. As required by state or local law, we provide a reasonable pay scale, including the hourly or salary range we reasonably expect to pay for this role.
