The Case Manager RN will oversee and manage resident care delivery across the entire continuum, from pre-admission through post-admission, ensuring appropriate care coordination and utilization of services. Key duties involve coordinating services, collaborating with teams to adjust care plans, monitoring resident progress, facilitating smooth transitions of care, and advocating for residents and families.
Candidates must have successfully graduated from an approved RN program and possess a current, unencumbered RN license for the state. Essential qualifications include the ability to make quick judgments, work independently, and maintain knowledge of nursing practices, regulations, and guidelines pertinent to long-term care.
