The Nurse Practitioner will deliver field-based primary care visits and physical exams directly in the patient's home, focusing on managing chronic diseases for high-risk patients. Responsibilities also include collaborating with an interdisciplinary team to ensure safe care transitions and treating acute needs to prevent emergency room visits.
Candidates must possess an Advanced Degree, specifically a Master’s Degree (NP), preferably focused on Adult, Geriatric, and/or Family practice areas. Required credentials include an active, unencumbered Pennsylvania NP license, Board Certification (ANCC or AANP), and reliable mobility via a valid driver's license and vehicle for local travel.
Your Impact: Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home. Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions. Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.
