The Medical Case Worker II supports Medi-Cal-eligible adults transitioning from incarceration by ensuring continuity of coverage, access to health services, and linkages to supportive resources like Enhanced Care Management. This role involves providing release planning services, coordinating with various county departments and community programs, and documenting client progress in medical charts.
Candidates must have either one year of experience as a Medical Case Worker or a bachelor's degree plus one year of professional casework experience involving interviewing, counseling, and assisting clients. Successful candidates must also clear the LA County Live Scan process and be willing to work in Correctional Health Facilities.
$24.33-$35.11 per hour
The Medical Case Worker II supports Medi-Cal–eligible adults involved in the justice system as part of California’s Medicaid 1115 JI Reentry Initiative. This role supports individuals transitioning from incarceration in state prisons, county jails, back into the community. The Medical Case Worker II works with clients during incarceration and post-release to ensure continuity of Medi-Cal coverage, access to comprehensive health services, medications, and supportive resources, including linkages to Enhanced Care Management (ECM). The position plays a critical role in improving health outcomes, supporting successful reentry, and reducing recidivism through coordinated, client-centered care.
