The Registered Nurse Case Manager provides professional nursing services to patients aged 18 and older based on physician orders, assessing needs, initiating plans of care, and evaluating their effectiveness. Key duties include providing patient comfort, coordinating nursing care, educating patients and families, and delegating appropriate tasks to staff while ensuring accurate documentation.
Candidates must minimally possess an Associate's Degree in Professional Nursing (ADN) and be licensed Registered Nurses in both Missouri and Kansas, holding current BLS certification. Essential requirements include a valid driver's license, proof of insurance, and basic proficiency with Microsoft Office applications, along with the ability to manage multiple tasks autonomously.

The Registered Nurse Care Team Leader (Case Manager) provides professional nursing services in accordance with physicians’ orders or plan of care to patients age 18 and older for a wide range of diagnoses. The RN, CTL is responsible for determining the nursing needs for an assigned group of patients/residents, initiating the plan, and evaluating the effectiveness of the plan of care. Supportive of Village mission and values. Completes training hours as required by policy.
The responsibilities described above represent the primary responsibilities of the job. Other responsibilities may be assigned by the supervisor as warranted by business needs. The incumbent is expected to perform all assigned responsibilities.
Education: This job minimally requires an associate's degree professional nursing (ADN)
May provide direction to care givers on a daily basis.
