The Navigator will provide clinical case management, rehabilitative services, and therapy within an Assertive Community Treatment model, focusing on developing recovery goals with participants. This includes conducting outreach, initial intakes, assessments, and acting as the team lead on substance use disorder issues.
Candidates must possess a minimum of two years of experience providing substance recovery services and two years of field experience working with populations dealing with serious mental illness, co-occurring disorders, or chronic homelessness. A valid California Driver's License, proof of insurance, and a clean DMV record are also mandatory.
Abode Services, one of the largest and effective nonprofits working to end homelessness in the Bay Area, is seeking a skilled a Health and Wellness Navigator to help support our Santa Clara County Programs in San Jose. **Please note this positions requires 2 years of experience providing substance recovery services.**
The Health and Wellness Navigator will provide clinical/therapeutic services, with focus on substance use disorders, on an Assertive Community Treatment team. This team will do outreach, engage, house, and provide wrap around clinical services for all residents. The residents are referred to by the County Office based on county data and other assessment processes as being the most frequent users of emergency services, criminal justice, and other homeless/county services. Program participants will have Serious Mental Health Disorders, Substance Use Disorders, and/or Chronic Health Conditions.
