The Advanced Practitioner will evaluate, interpret, and coordinate information to make decisions and recommendations regarding patient care across various settings including homes, clinics, and nursing homes. Responsibilities include managing an average of 8-10 patients daily with shared, telephone-only call duties outside of standard Monday-Friday clinic hours.
Candidates must be a Nurse Practitioner or Physician Assistant holding an unrestricted medical license in Iowa and possessing current prescribing authority. Experience in post-acute and/or long-term care facilities is preferred, and Palliative Care certification is desired.
Full & part-time roles Clinic hours: Monday-Friday 8am-5pm 4 day work week No nights, weekends, or holidays Palliative Care services provided in patient's homes, clinic and/or nursing home setting Average of 8-10 patients per day; no nights or weekends Shared call (telephone only) amongst the providers. Currently 1:17 – Triaged through MyNurse Evaluates, interprets, and coordinates information to make decisions and recommendations regarding patient care Patient-focused, friendly, and willing to collaborate in a team environment Excellent communication (written and verbal), analytical and problem-solving skills
