Specialty Nurse RN - Breast Care Center - Full Time - Days

Job summary

• Provides direct and indirect patient care in the office setting to patients, families and caregivers in the Medical Group practice.
• Uses the nursing process consistent with acceptable standards of the profession and MVHS Medical Group.
• Provides high quality patient centered care that reflects initiative, flexibility and responsibility with a minimum amount of administrative supervision.
• Determines priorities of care based on physical and psychosocial needs, as well as factors influencing patient flow through the system.
• Communicates with outpatient clinic physicians and NP/PAs (healthcare providers) about changes in patient’s status, symptomatology and results of diagnostic studies.
• Is able to respond quickly and accurately to changes in condition or response to treatment

• Core job responsibilities
• Support the PCP in the delivery of care by gathering and documenting relevant clinical data, as appropriate and per patient need.
• Participate in the daily huddle to identify individual patient needs for upcoming appointments.
• Clinical liaison between patient and provider for questions, concerns and testing results.
• Communicate effectively with other members of the care team in coordinating the individualized plan of care.
• Support PCP during in office procedures and examinations.
• Provide outreach and education to patients in need of services as identified through quality improvement initiatives (Population Health Management).
• Perform duties within scope of practice to the highest level of licensure.
• Provide direct support & guidance to other members of the care team as appropriate.
• Able to perform assessment data collection on all patients utilizing tools / forms in EMR.
• Adequately assess and reassess pain. Educate the patient and family regarding pain management.
• Provide age appropriate care to patients in the community setting.
• Consistently document procedures completed in the EMR, to include medications administered and follow-up assessments
• Deliver medication utilizing the five (5) rights and observes patient for desired untoward effects.
• Maintain current knowledge of medications and their correct administration.
