The Licensed Practical Nurse will administer psychotropic medications, monitor client health status, and document all care using the Welligent EMR platform. Responsibilities also include collaborating on individualized treatment plans and providing crisis intervention services as part of a rotational on-call schedule.
Candidates must possess an active, unrestricted LPN license in North Carolina and have reliable transportation, vehicle insurance, and a valid driver's license to travel within the community. Experience in mental health or substance use services is preferred, along with strong communication and relationship-building skills.
We are seeking a full-time Licensed Practical Nurse (LPN), Behavioral Health to join our Assertive Community Treatment (ACT) team and provide compassionate, community‑based care to adults with significant mental health and substance use needs.

As an LPN on our ACT Team, you will be an essential part of a multidisciplinary team providing intensive, wraparound behavioral health services. You’ll deliver medication support, clinical interventions, education, and crisis response that help individuals achieve stability, independence, and improved quality of life.
In this role, you will build meaningful relationships, help clients navigate real‑life challenges, and ensure continuity of care across community settings. You’ll work closely with psychiatrists, therapists, co‑occurring disorder specialists, and other mental health professionals to support truly holistic treatment.

This full-time LPN role follows a Monday through Friday schedule with rotating on‑call responsibilities. Travel within the community is required.
