The RN Care Coordinator oversees patient care progression and discharge planning to ensure health needs are met while promoting quality and cost-effective outcomes. They collaborate with multidisciplinary teams, including physicians and insurers, to advocate for the patient and address barriers to care transitions.
Candidates must hold a nursing diploma or associate degree, with a BSN preferred, and possess a valid RN license. A minimum of five years of total nursing experience is required, including at least two years of acute hospital clinical experience.
Variable (Must be open to work one weekend shift per week if needed). Job Requirements In addition to bringing your whole self to the workplace each day, qualified candidates will need the following: Graduate of an Accredited School of Nursing Nursing Diploma or Associates Degree in Nursing required, BSN preferred RN License in Applicable State of Employment required Minimum two (2) years of acute hospital clinical experience or a Master's degree in Case Management or Nursing field in lieu of acute experience. At least five years of total nursing experience required. Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification preferred. Experience working with EMR preferred. Working knowledge of regulatory requirements and accreditation standards strongly preferred. BLS from the American Heart Association required within three months of hire if located within hospital Physical Requirements - Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)
