The hospice medical social worker will be responsible for conducting psychosocial evaluations, providing ongoing counsel, offering bereavement services, and engaging in community education outreach. They must deliver physical, emotional, and spiritual support to patients and families in accordance with the established plan of care.
Candidates must possess a heart to serve patients and families, requiring an MS degree in social work from an accredited school and current state social worker licensure. A minimum of two years of clinical work experience, preferably in healthcare or hospice, along with reliable transportation, is mandatory.
