The Ambulatory Care Nurse provides nursing care management services to assigned populations using a collaborative approach to coordinate the patient's plan of care and maximize community living ability. This role involves identifying at-risk patients requiring care management and acting as a communication link between the physician, patient, family, healthcare team, and community resources.
Candidates must be a Registered Nurse with a minimum of 8 years of related nursing experience and a Bachelor's Degree in Nursing. The nurse is expected to support organizational mission and adhere to professional nursing practice standards and models like PFCC.
