The Physical Therapist will deliver skilled therapy intervention and treatment to patients in a home setting, utilizing appropriate treatment plans and assessment skills. Responsibilities also include monitoring patient progress toward goals, ensuring compliance with physician's orders, and educating patients and caregivers for appropriate discharge.
A valid Physical Therapy License is required, and the company is accepting new graduates for this position. Candidates must also possess a valid Driver's License and demonstrate good oral, written communication, and documentation skills.
$5,000 Sign-On Bonus Available for Eligible Full-Time Physical Therapists

We are seeking skilled and compassionate Physical Therapists (PTs) to join our growing team across the Bay Area. The ideal clinician is patient-centered, collaborative, and committed to helping patients restore mobility, strength, safety, and independence in the home environment.
As an AlphaCare PT, you will evaluate and treat patients recovering from illness, injury, surgery, or functional decline. You will develop individualized treatment plans, deliver evidence-based therapy interventions, and collaborate closely with physicians, nurses, caregivers, and interdisciplinary team members to support coordinated, outcome-driven care. Evaluate patients in the home setting and develop individualized treatment plans based on physician orders Provide skilled physical therapy interventions focused on mobility, strength, balance, endurance, and safety Monitor patient progress and modify treatment plans as appropriate Educate patients and caregivers on therapeutic exercises, mobility strategies, fall prevention, and safe home practices Assess functional limitations and support patients in achieving independence goals Collaborate with physicians, nurses, and interdisciplinary care teams to coordinate patient care Participate in case conferences and care planning discussions Maintain accurate, timely, and compliant clinical documentation Communicate changes in patient condition and treatment progress proactively Support safe discharge planning and continuity of care Participate in continuing education and quality improvement initiatives Deliver care in alignment with AlphaCare’s commitment to patient time, multilingual community care, and clinical reliability

Work Location In-person home visits throughout assigned San Francisco, Peninsula, East Bay, and San Mateo territories
Pay Range $65.00 – $75.00 per hour + $5,000 Sign-On Bonus
