The Case Manager provides essential case management and care coordination services, ensuring the continuum of care and optimal resource utilization for patients in treatment. This role involves coordinating assessments, treatment planning, crisis intervention, facilitating community resource linkages, and managing discharge activities.
A Bachelor’s degree in a Human Service field from an accredited college or university and a valid/current Driver's License are required for this position. The role may also require the ability to travel up to 15% of the time.
