Part-Time Registered Nurse (RN) - San Diego

Location

• Various patient homes throughout San Diego County (including El Cajon, La Mesa, Chula Vista, etc.).Schedule: Part-Time; consistent assignment potential based on patient needs and RN availability.
• Reports To: Clinical Supervisor

Core Responsibilities

• Perform comprehensive skilled nursing visits according to physician orders and the established plan of care.
• Conduct initial patient assessments, ongoing monitoring, and necessary interventions.
• Manage and administer medications and treatments safely and accurately.
• Provide thorough patient and family education regarding diagnoses, treatment plans, and self-care management.
• Maintain accurate and timely documentation in the electronic health record (EHR) system.
• Collaborate with the home health team (PT, OT, HHA, etc.) to ensure coordinated care.

Essential Qualifications

• Active, unrestricted Registered Nurse (RN) license in the State of California.
• Minimum of one (1) year of recent clinical experience (Home Health experience is a strong plus!).
• Current BLS/CPR Certification.
• Must have reliable, insured transportation to travel to patient homes within San Diego County.
• Strong foundation in clinical assessment and professional communication.
