The Nurse Practitioner will autonomously provide primary medical care to patients requiring Palliative or Hospice interventions, which includes rounding in facilities and updating care plans in collaboration with physicians. Responsibilities also involve assessing patients, reviewing medications, creating care plans, and discussing advanced directives and goals of care with patients and caregivers.
Candidates must possess a current state Board of Registered Nursing license and a Master’s Degree from an accredited Nurse Practitioner’s Program, along with National Certification or eligibility within six months of employment. A minimum of one year of Nurse Practitioner experience and previous Hospice experience are preferred, alongside reliable transportation and the ability to pass background checks.
