The RN Case Manager provides support through the continuum of care from admission to discharge. They coordinate utilization management and discharge planning to enhance clinical outcomes and manage the cost of care.
Requires a Bachelor's Degree in Nursing and at least one year of acute care hospital experience. A valid California RN license and AHA Basic Life Support certification are mandatory.
Responsible for providing support and expertise through a continuum of care from admission through discharge. Supports the physician and interdisciplinary team to facilitate patient care, while enhancing the quality of clinical outcomes, patient satisfaction and managing the cost of care. The role integrates and coordinates utilization management, care facilitation and discharge planning functions. The RN Case Manager incorporates admission status and medical necessity determination, to ensure that appropriate care and services are delivered efficiently and timely.
