The APP will conduct comprehensive health assessments, develop individualized treatment plans, and provide patient education. They will also collaborate with healthcare team members to ensure coordinated patient care.
Candidates should have 1-2 years of clinical experience in a primary care or internal medicine setting and hold an active PA or NP certification. An active, unrestricted CT medical license is required, along with ACLS/BLS/PALS certifications.

As a valued member of our team, you will: Conduct comprehensive health assessments, including obtaining detailed medical histories, performing physical examinations, and ordering/interpreting diagnostic tests. Develop and implement individualized treatment plans in collaboration with the supervising physician, focusing on health promotion, disease prevention, and management of acute and chronic conditions. Prescribe medications, order therapies, and manage medical devices according to state regulations and collaborative practice agreements. Provide patient education and counseling regarding health maintenance, disease management, medication adherence, and lifestyle modifications. Evaluate and manage common acute illnesses (e.g., upper respiratory infections, influenza) and chronic conditions (e.g., hypertension, diabetes, asthma). Perform minor office procedures as trained and within scope of practice. Collaborate effectively with physicians, nurses, medical assistants, and other members of the healthcare team to ensure coordinated and comprehensive patient care. Document all patient encounters accurately and thoroughly in the electronic health record. Participate in quality improvement initiatives and adhere to all organizational policies and procedures. Serve as a resource for medical staff and participate in educational activities as needed. Provide age-appropriate care to patients across the lifespan, from infants to geriatrics. Manage referrals to specialists and coordinate follow-up care. Maintain current knowledge of family medicine best practices and engage in ongoing professional development.

Active, unrestricted CT medical license or in process of obtaining ACLS/BLS/PALS (as required) Certification DEA License Schedule: 40 scheduled hours, M-F, 8am-5pm, 4-day work week encouraged Skill Set: Prefer 1-2 years in a clinical working experience as a NP/PA within the given specialty/service line recommended. APRNs, PAs, CRNAs and CNMs are required to be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process.
