The primary purpose involves planning, organizing, developing, and directing Physical Therapy Services to maintain the highest degree of quality care according to regulations. Responsibilities include coordinating patient care plans, briefing patients on procedures, determining proper equipment usage, and recommending therapy program modifications based on progress.
Candidates must possess a current and valid Physical Therapy license and CPR certification, and must pass a pre-employment background check and drug test. One year of experience in a skilled nursing facility is preferred, along with the ability to communicate effectively in English and maintain confidentiality.

Meet with administration, medical and nursing staff, as well as other related departments in planning therapy services-to include patients and family Assist in developing, implementing and coordinating patient care plans for individual patients in coordination and conjunction with the patients assessment Brief patient of procedures involved in physical therapy and prepare necessary equipment-reassure patient before and during therapy treatment Determine proper equipment usage, application and body position, make adjustments when necessary in the administration of physical therapy treatments Recommend modifications of changes in the patient’s therapy program based on evaluation of progress Ensure that all therapy notes are informative, descriptive and follow Company’s policy and procedures About Us The Center at Centennial is a skilled nursing facility that serves patients needing physical rehabilitation and/or complex nursing care. Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents. Must be able to relate information concerning a resident’s condition. Must be knowledgeable of HIPAA compliance and requirements, and have the ability to maintain highly confidential information. Must not pose a direct threat to the health and safety of other individuals in the workplace.
