In this role, you will provide high‑quality direct patient care under the supervision of a Registered Nurse (RN).
You will follow individualized care plans, administer treatments, document clinical information, and collaborate with the interdisciplinary care team to support positive patient outcomes.
Key Responsibilities Provide direct patient care in accordance with the State Nurse Practice Act.
Carry out the plan of care developed by the Registered Nurse.
Complete accurate, timely, and compliant clinical documentation.
Perform patient observations and collect assessment data to report to the RN or physician.
Communicate the need for additional home health services (HHA, OT, PT, MSW, ST, Dietician) to the clinical team.
Utilize medical equipment and supplies safely, efficiently, and according to agency guidelines.
Participate in ongoing professional development and skills training.
Qualifications Current LVN license in California.
Minimum of 1 years nursing experience preferred (home health or medical/surgical experience strongly valued).
Knowledge of professional nursing standards and clinical best practices.
Strong verbal, written, and organizational communication skills.
Reliable transportation for home visits; valid driver’s license and current auto insurance required.
Current CPR certification.
Ability to lift, position, and transfer patients; requires frequent standing, walking, bending, and reaching.
