include conducting annual wellness visits and managing high-risk patients primarily via telehealth, with potential for in-home visits, while thoroughly documenting medical information and administering necessary tests. The role also requires collaborating with the Primary Care team, ensuring timely record completion, providing patient education, and maintaining strict HIPAA confidentiality.
Candidates must possess a current Utah Nurse Practitioner license and certification from ANCC/AANP, along with at least two years of clinical experience in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care. A current DEA license, prescriptive authority, and active CPR/BLS certification are also mandatory.

This is a full-time Nurse Practitioner opportunity (5 days a week, 8am-5pm schedule). This role will mostly involve telehealth visits but can include in-clinic and in-home visits. Home visits will be scheduled within a 30 mile radius of your home region. This position will be required to flex across regions to provide telehealth coverage as needed to accommodate for scheduling changes.
