The Case Manager supports young adults in securing and maintaining stable housing by conducting intake assessments, developing individualized service plans, and connecting clients to essential resources like education and employment. Responsibilities also include coordinating medical and mental health services, advocating with external agencies, and performing basic medical monitoring tasks such as recording vital signs.
Candidates should possess 1 to 2 years of case management experience working with populations experiencing homelessness, mental illness, or trauma, and ideally have a Bachelor's degree in a related human services field. Required abilities include proficiency with Microsoft Office, excellent organizational skills, the capacity to work independently, and the physical ability to lift up to 50 pounds and travel frequently.
The Supportive Housing Case Manager - CNA (Ages 18–25) supports young adults in securing and keeping stable housing while building life skills and independence. Using a positive, youth-centered approach, you’ll carry a small caseload (12–18 clients), offer flexible one-on-one guidance, and connect them to education, jobs, and community resources. This is a rewarding role for anyone who enjoys cheering young people on as they step confidently into adulthood.
Monday - Fridays (12am-8am) Sat and Sun (12am-8am) Key Responsibilities Conduct intake assessments, develop individualized housing stability/service plans, and update plans quarterly or as needs change. Assist clients in securing and maintaining permanent housing (lease signing, landlord mediation, rental arrears negotiation, unit inspections, etc.). Coordinate medical, mental health, substance use treatment, benefits enrollment (SSI/SSDI, SNAP, Medicaid), employment, and other supportive services. Use harm-reduction and trauma-informed approaches in all client interactions. Advocate for clients with landlords, courts, hospitals, and benefit agencies. Facilitate crisis intervention and de-escalation when needed; participate in on-call rotation (if applicable). Participate in case conferences, staff meetings, and regular supervision. Collaborate with property managers, housing navigators, peer specialists, and clinical teams. Monitor and record vital signs, including temperature, pulse, and blood pressure. Provide support in urgent care situations and assist with basic medical procedures. Help patients with mobility needs, including lifting and transferring individuals as necessary. Communicate effectively with staff regarding patient conditions and any changes observed. Provide empathetic assistance to patients and uphold safety standards Participate in patient care planning and collaborate with healthcare professionals to ensure comprehensive care.
