The Physical Therapist is responsible for direct patient care, professional development, outcome assessments, and ensuring compliance with regulatory standards to achieve organizational values. This role involves acting as a case manager to evaluate patient needs, providing skilled therapy and teaching in the home, and coordinating resources for a comprehensive care plan.
Required qualifications include a Bachelor's, Master's, or Doctorate degree in Physical Therapy and a current license with the Minnesota Board of Physical Therapy. Preferred qualifications include at least 3 years of experience as a Physical Therapist, preferably in homecare, and proficiency with EMR systems and OASIS data sets.

The Physical Therapist is responsible for assuring that the PNHS values of Head + Heart, together, are achieved through daily departmental operations. This includes, but is not limited to, direct patient care, professional growth and development, outcome/compensatory assessments, compliance with regulatory standards and participating in process improvements to advance patient care outcomes and satisfaction.
The Physical Therapist will communicate in a timely, clearly, and professional manner with staff, patients, patient family members, physician, and other internal/ external customers. The staff will demonstrate skills in listening, conflict management, system problem solving, decision making and process improvement. Therapists will practice in compliance with professional standards of practice and professional code of ethics.
The Homecare Physical Therapist acts as a case manager to access and evaluate patient care needs and provides skilled therapy care and teaching to patients and families in their home. The Physical Therapist is responsible to improve and prepare patients and families for self-directed and/or assisted care in their community setting. The Physical Therapist coordinates medical, social, and financial resources to develop a comprehensive plan of care. The therapist supports the following community care processes: admission, discharge, referrals to other disciplines, billing, and care delivery.
