The Nurse Practitioner facilitates comprehensive, individualized palliative care by providing diagnosis, treatment, consultation, and follow-up under the direction of the Medical Director. Responsibilities include managing the patient's palliative care plan from initial assessment through discharge, addressing symptoms, and coordinating with the interdisciplinary team.
Candidates must possess a minimum of a Master of Science in Nursing and two years of Nurse Practitioner experience, preferably specializing in Family or Adult Medicine, with proficiency in Part B billing. Essential requirements include maintaining current state licensure, certification, and securing a collaborative agreement with a supervising physician or the Palliative Medical Director.
