The Social Worker will complete initial psychosocial assessments, establish individualized plans of care, and provide emotional support, education, crisis counseling, and advocacy for patients and families during the end-of-life journey. Responsibilities also include coordinating community resources, communicating with partners, participating in team meetings, and maintaining accurate documentation according to the Social Work Code of Ethics.
A Bachelor's in Social Work (BSW) from an accredited institution is required, or a bachelor's degree in a related field plus one year in healthcare; a Master's of Social Work is preferred. Candidates must be certified or licensed by the State of Wisconsin and possess a valid Wisconsin Driver's License with reliable transportation.

As a Social Worker, you will support patients and families through the end-of-life journey as part of an interdisciplinary team. Responsibilities include psychosocial assessments, individualized care planning, emotional support, counseling, education, and advocacy. You will coordinate resources and referrals, assess bereavement needs, participate in care planning and IDT meetings, assist with funeral planning as requested, support Volunteer Services and specialty programs, maintain accurate documentation, uphold the Social Work Code of Ethics, and participate in a 24/7 coverage rotation.

Bachelor’s in Social Work (BSW) from an institution accredited by the Council on Social Work Education; or bachelor’s degree in psychology, sociology, or other fields related to social work along with one year in a healthcare setting. Master’s of Social Work preferred. Certified or licensed as a social worker (CSW, CAPSW, CISW, or LCSW) by the State of Wisconsin required. Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage.
