Physical Therapist- Senior Living Home Health Visits

Physical Therapists will make home visits to clients, performing diagnostic tests, implementing protective interventions, and developing individualized care plans including therapeutic exercises and gait training. They must continually assess and revise care plans, participate in multidisciplinary planning, and educate clients and family members on necessary rehabilitative activities.

Candidates must possess a current Pennsylvania Physical Therapist license and a minimum of one year of recent clinical experience as a Physical Therapist. Graduation from an approved Physical Therapy program is required, along with the ability to work independently and strong computer skills, with prior Electronic Medical Records experience preferred.
