The coordinator will support the care of pregnant patients with diabetes by tracking, monitoring, and providing outreach and support within the Endocrinology and OB teams. Essential functions include serving as a patient contact point, coordinating care, assisting with treatment plans, and ensuring timely CGM initiation and follow-up.
The role requires excellent communication, interpersonal, and organizational skills, with familiarity with EHR and spreadsheet software being preferred. Preferred experience includes prior authorization workflows, data collection/tracking, and working in ambulatory or outpatient healthcare settings.
The ideal candidate will work with the Endocrinology and OB teams to support the care of pregnant patients with diabetes, and assist with tracking, monitoring, outreach, and supporting all patients involved in this program.
Schedule: Monday-Friday, 9AM-5PM with options for some remote access
Significant (more than 50%) portions of daily assignments involve application of manual skills requiring motor coordination in combination with finger dexterity.
Some (less than 50%) portions of daily assignments involve prolonged standing, transporting patients, material, and equipment, or lifting, moving, or carrying heavy (over 30 lbs.) materials for intermittent periods throughout the day.
Some (less than 50%) portions of daily assignments involve exposure to dirt, odors, noise, or some work is performed with exposure to temperature/weather extremes. Working surfaces may be unlevel, slippery, or unstable.
Work may require evening, night, holiday, or weekend assignments on occasion.
Some (less than 50%) portions of daily assignments involve occupational risk, such as cuts, burns, exposure to toxic chemicals, injuries from falls, or back injury sustained with assisting in moving/lifting/positioning patients, equipment, or materials.
OSHA Category III - Exposure not anticipated. Work does not involve exposure to blood, body fluids, or tissue, and does not require performing unplanned exposure tasks.
Job does not require the use of safety equipment, such as HEPA masks, for prevention of infection.
Significant portions (more than 50%) of daily assignments involve ergonomic risk.
